Town of Youngtown - Passport Acceptance Facility
The Town of Youngtown Passport Acceptance Service processes only new applications, long-time (more than 5 years) expired applications and youth applications.
It is required that you please bring all older passport(s) to your appointment.
Forms must be printed single sided and should be completed in black in.
We do not process passport renewals at this time. For renewal information, please visit the State Department website at https://travel.state.gov.
The passport acceptance service is available via appointment only.
Please call Town Hall at 623-933-8286 to schedule an appointment.
If you are 15 minutes late to your appointment or you do not have the appropriate documentation, you will need to reschedule the appointment.

Requirements For Passports

Complete all necessary forms.
Forms must be printed single sided and should be completed in black ink.

Provide proof of citizenship (original and a copy).
Certified birth certificate, passport, or naturalization certificate.
- For minors 15 and under, child and both parents must be present.
- For minors 16-17, child and one parent must be present.

Provide proof of identity (original and a copy).
Valid government issued picture ID.
- For minors, parents must present valid ID.

Provide a passport photo.
- Please bring at least one passport photo with you,.
